Our tour guides went above and beyond. They were very knowledgeable and accommodating (they'll provide alternative routes in case you're worried about jumping into the water). It lasted closer to 7 hours because we were picked up in Taormina at and picked up one more group, stopped in a nice cafe before and after river trekking, and made an extra stop in the park (which was a cool add-on). We were picked up at our hotel around 8:30am and returned around 3pm. There's some climbing involved, but it's very doable for moderately active people. Just wear your bathing suit and bring a change of clothes, a towel, water and snacks - they'll have everything else. If you like adventure this is a must-do!
